You might be eligible for financial assistance and valuable support services. Let's find out together!

You may be pre-qualified!
  • Currently on the CCSP or SOURCE Program
  • Medicaid eligibility or enrollment
  • Experiencing a need for assistance due to a severe medical condition
  • Receiving Supplemental Security Income (SSI), Social Security Disability Income (SSDI), or social security income
  • Biological relation to the family member you want to take care of you
  • Lives in Georgia

*  Keep in mind: Medicare alone does not meet the enrollment criteria for this program due to government regulations.

  • Must be at least 18 years of age
  • Must be related biologically or through marriage, such as a daughter, grandson, daughter-in-law, or son-in-law
  • Must pass a criminal background check
  • Must reside with your loved one as the primary caregiver
  • Cannot be employed outside of taking care of the loved one
